If we never showed up to claim our title (and birthright), we may be the "unknown owner". In Illinois, the court clerk is the trustee of all unknown owners.

Unknown owner - Jane Doe - John Doe - DOE - Department of Energy

Surname, First Second - is correct - is the creditor (is missing).

If unknown/missing for too long it will end up looking like this:
SURNAME, First Second
Then like this: SURNAME, FIRST SECOND
Like a tombstone.

First Middle Last - is a debtor
First Last - is a debtor
FIRST (MIDDLE) LAST - is a debtor
